Everyday moments · Adult information
I am worried I will
forget what to say.
A few notes can help you explain what matters.
Bringing Minds Into Sync
A real-life situation · Illustrative example
The appointment starts. Your mind goes blank.
You have waited to explain your difficulties. In the moment, you lose your words. Afterwards, the question comes back.
You do not need a perfect speech. Bring a few notes you can return to.
Three lines to prepare
Use your own experience. These examples are not answers to copy into an assessment.
What happened?
“I missed two appointments even though I meant to attend.”
What was the impact?
“I lost time from work and felt worried about doing it again.”
What do I want to ask?
“What options or next steps could help?”
Add how often it happens and when it began, if you know. You can say when you are unsure.
If writing feels too much
Make a private voice note, or ask someone you trust to help you jot down one main point.
Ask the clinic in advance about communication adjustments or a person joining you.
Words you can use
“I sometimes lose track of what I want to say. Can I refer to my notes, and can we write down the next steps?”
Check the appointment instructions
Your notes do not replace requested forms, records or health readings. Assessment and medication appointments can need different preparation.
Check which medicines, previous reports and other information you need to provide. If something is missing, contact the clinic for guidance. Use the secure route the clinic provides for medical records.
For a video appointment, check your device, sound and connection beforehand.
If you need more help preparing
Tell the clinic what would make the appointment easier. You can ask for questions to be explained or repeated. Agree any appointment recording with the clinician beforehand.
Before the appointment ends
Check what happens next, who will arrange it, and who to contact with a question.
If medication is discussed, ask about benefits, risks, alternatives, monitoring and costs. An appointment does not guarantee a diagnosis or prescription.
Forgot something afterwards?
Use the clinic’s agreed contact route to ask how to share an important missing detail. If you need urgent medical help, do not wait for a routine reply.
